Incident Analysis and Proactive Risk Assessment

This 1.5-day live virtual workshop teaches health care professionals the following:

Day 1: How to investigate patient safety incidents in health care with a system-level approach (incorporates human factors engineering principles) using root cause analysis.

Day 2: How to identify potential failures in a system and redesign processes to mitigate risk using failure mode and effects analysis.

  • Dates:    Day 1: 9 a.m. to 3:30 p.m.; Day 2: 9 a.m. to 1 p.m.
  • Location: Join us virtually from your own location – virtual link will be emailed once registration is processed
  • Cost:    $850 per person, plus applicable taxes (online payment only)
  • Requirements:   Computer with microphone, webcam, speakers, and internet connection; lined paper and pen; wall space to use for hands-on-practice
