Primary Care Networks Program Management Office

Primary Care Networks bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. PCNs are comprised of groups of family physicians working with other health care professionals such as nurses, nurse practitioners, dietitians, pharmacists, social workers and mental health professionals. PCNs develop solutions to meet the needs of the local community and their health issues. PCNs are created through an agreement between physicians and Alberta Health Services.

What is the Primary Care Networks Program Management Office?

The Primary Care Networks Program Management Office provides operational and business administrative support services to Primary Care Networks, as well as tools and resources to support PCNs in communications, evaluation and quality, and business planning. We also work closely and collaboratively with the other Alberta Medical Association programs such as Practice Management Program and Toward Optimized Practice to support PCNs.

Support for Patient’s Medical Home transformation is managed through three AMA programs. PCN PMO works in collaboration with PMP and TOP in developing tools, resources, training and support for this effort.
