Primary Care Networks Program Management Office (PCN PMO)

Primary Care Networks (PCNs) bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. PCNs are comprised of groups of family physicians working with other health care professionals such as nurses, nurse practitioners, dietitians, pharmacists, social workers and mental health professionals. PCNs develop solutions to meet the needs of the local community and their health issues. PCNs are created through an agreement between physicians and Alberta Health Services.

What is the Primary Care Networks Program Management Office (PCN PMO)?

The Primary Care Networks Program Management Office (PCN PMO) provides operational and business administrative support services to Primary Care Networks (PCNs), as well as tools and resources to support PCNs in communications, evaluation and quality, and business planning. We also work closely and collaboratively with the other Alberta Medical Association programs such as Practice Management Program (PMP) and Toward Optimized Practice (TOP) to support PCNs.

Additionally, the Primary Care Network Evolution initiative is managed through the PCN PMO. We provide opportunities to collaborate with all other support programs such as PMP, TOP and AIM in developing and delivering tools, resources, training and support for this initiative.
