Susan Haggarty is a clinical pharmacist with the System Case Manager/Geriatric Consult Team in Edmonton Zone Home Living. She holds a B.Sc. (Pharmacy) from the University of Alberta and a B.Sc. (Biomedical Toxicology) from the University of Guelph. She has contributed to various medication management initiatives as the co-chair of the Edmonton Zone Home Living Medication Management Task Group and as an active member of other local and provincial committees.

Dr. Jasneet Parmar is a Care of the Elderly physician and has worked for the Specialized Geriatrics Program, since 1992. She is an Associate Professor, Department of Family Medicine, University of Alberta. She is the Medical Lead of Home Living and Transitions, AHS Edmonton Zone. Jasneet has been very active in developing and implementing clinical programs for the care of the elderly with complexity and frailty. Her research currently is focused on supporting family caregivers of older adults by the healthcare system.
