Purpose
Building on existing member organization strategic directions, the Coalition’s goal is to foster a coordinated and aligned approach to meet the needs of patients, caregivers, families, primary health care teams, Primary Care Networks (PCNs) and the healthcare system today and into the future.
The CPHC evolved from the Primary Care Alliance (PCA) in 2023 and has since grown to being a revolutionary table within Canada that includes several multidisciplinary stakeholders who are committed to building a common vision of delivery models, structures and support required to advance primary health care.
The current work of the CPHC is focused on bringing primary care providers and partners together to advance team-based care in the province of Alberta.
