Dental pre-op and related services

Pre operative history and physicals (03.04M) are only billable to AH if the anesthetic portion of the procedure is an insured service (GR 10).

If the anesthetic portion of the procedure is not an insured service, the pre operative history and physical are billable as an uninsured service to the patient.
