Goals of Care Forms or “Green Sleeves”

Wondering what to bill when you complete the Goals of Care form, also known as the “Green Sleeve”?

Although this service is not billable on its own to the patient or to Alberta Health, the time spent completing the form can be claimed using complex modifiers (CMGP/CMXC) as appropriate.

Complex modifiers can be added to the appropriate visit service and are used for time spent providing patient care and management on the same date of service as the visit occurred.
