Transition Planning Mini-Series

This is a two-part Transition Planning mini-series within the Alberta Children’s Hospital Complex Care ECHO, focused on supporting adolescents and young adults with medical and behavioral complexity as they transition from pediatric to adult care.

This mini-series is designed for family physicians, community pediatricians and other interdisciplinary team members/providers, and will explore the practical, real-world strategies for early transition planning, shared care and effective handover between pediatric and adult providers. 
 
Attendance at both sessions is strongly encouraged, as the clinical case presented in Session 1 will inform interactive discussions and an applied learning activity in Session 2. 
 
Session 1 – Principles of Transition Planning
May 14 | 6:00–7:45 PM
This session will focus on early preparation for transition, including capacity assessments, guardianship and trusteeship considerations, funding and support timelines and the lived experience of families navigating the transition to adult care.
 
Session 2 – Shared Care & Transfer of Care
June 4 | 6:00–7:45 PM
This highly interactive session will build on the case introduced in Session 1 and explore shared care models between pediatricians and family physicians, communication strategies at transfer of care and practical approaches to building capacity in adult care settings through facilitated breakout discussions and hands-on activities.
 
To attend you must register for the ACH Complex Care ECHO program (registration covers all sessions, you may choose to attend those that are most relevant to you and your practice). 

Both sessions are eligible for 1.5 hours of CME MOC Section 1 credits each and Session 2 will follow the PPIP – Physician Practice Improvement Program Template.
