H2H2H Transitions
Home to Hospital to Home (H2H2H) transitions are a key set of actions designed to ensure the safe and effective coordination and
continuity of care as patients shift between the Patient's Medical Home and hospital.
The Primary Health Care Integration Network, along with primary care and community health service providers, specialists, patients and families have developed a H2H2H Transitions Guideline to identify best practices in facilitating effective H2H2H transitions of care.
The Health Neighbourhood
The Health Neighbourhood includes essential services integral to the health journey, including:
- Patient's Medical Home
- specialists
- hospitals
- laboratories
- emergency medical services
- home care
- continuing care
The Alberta Medical Association supports Primary Care Networks (PCNs) in promoting the Health Neighbourhood as the framework for the Patient's Medical Home (PMH).
The PMH is where individuals feel most comfortable discussing personal and family health concerns. As part of our contribution to the Primary Health Care Integration Network, we gather input from physician leaders and develop content and processes to improve transitions between PMH and Health Neighbourhood services.
