What is the PFSP's Case Coordination Service?
The PFSP’s Case Coordination service supports individuals to access a variety of clinical and non-clinical resources that are necessary to meet an individual’s needs. While the Case Coordination service does not act in a treatment capacity, the Case Coordination team can, with consent, liaise with treatment providers and other involved individuals. The goal of the Case Coordination team is to help physicians, resident physicians, and medical students find optimal health and have access to the supports they need to succeed in their professional and personal lives. Participation is voluntary.
Your privacy is treated with the highest regard by the Case Coordination team. When communicating and coordinating with third parties, informed consent is obtained from you. You can revoke consent at any time during the process and / or request that your Case Coordination file be closed.
In a situation where the Case Coordinator was obligated by law or regulatory standards, they would be required to take reasonable steps to ensure the safety of the caller and the safety of others. This could include reporting to the appropriate authorities. In this case, these concerns would be discussed with the caller transparently with the goal to come up with a plan that the caller is comfortable with.
Working with the Case Coordination team requires transparency so you can receive the support you require. If you have concerns related to the limits to confidentiality, the Case Coordination team will work with you to understand your options.
Limits to Confidentiality
In a situation where the Assessment Physician or Case Coordinator was obligated by law or regulatory standards, they would be required to take reasonable steps to ensure the safety of the caller and the safety of others. This could include reporting to the appropriate authorities. In this case these concerns would be discussed with the caller transparently with the goal to come up with a plan that the caller is comfortable with.
The Case Coordination team will work with you throughout the decision-making process
to ensure transparency, and:
- Assist, support and implement a plan that will help you access the support you need, including communicating with third parties with your informed consent, and suggesting resources or referrals to other professionals such as licensed therapists and treatment facilities.
- Make suggestions based on your needs and the experience of the Case Coordination team, but you will make the final decisions regarding your well-being.
- Provide ongoing support until you feel you no longer require it. Initially, your case may require more intensive involvement through several online meetings, with less frequent contact over time.
- Meet with you over a virtual platform (e.g., Zoom) and be available to you, typically by email, when you need them.
To maintain confidentiality of all Case Coordination clients, the Case Coordination team expects you will:
- Provide the information and consent necessary for the team to work effectively on your behalf.
- Communicate in a timely manner and agree to meet as needed.
- Provide the team with at least one week’s notice to apply for funding on your behalf for an assessment, treatment program, or monitoring of health conditions.
- Adhere to the treatment program you have chosen. Comply with the Standards of Practice of the College of Physicians & Surgeons of Alberta.
The Clinical and Program Co-Directors of PFSP (or designate) may apply for Compassionate Assistance Program (CAP) funding on your behalf, if you are eligible. CAP funding provides financial assistance for eligible clients who need an independent assessment, treatment or monitoring for health conditions that have the potential to impact their ability to practice or to continue medical training, and for whom the cost of the appropriate service would cause significant financial hardship. In some circumstances, the costs may be shared with you. Case Coordination participants also have access to an increased number of therapy hours on a yearly basis that is at no cost to them.
