How to Apply for the PCPCM
-
Step 1 | Review PCPCM Resources
These resources will help you determine if the PCPCM is right for you and offer guidance on the enrollment process, changes to billing, panel management and more.
Before applying, physicians and teams should explore the following supports:- PCPCM Operations Manual
- PCPCM Essentials Guide
- PCPCM In-Basket Fee Codes
- PCPCM Financial Calculator - Member login required
- PCPCM FAQ
- Alberta's Physician Compensation Comparison Table
- PCPCM Information Sessions - on demand
- PCPCM Panel Prep Webinar Series - on demand
-
Step 2 | Meet Pre-Requisites
To be eligible for the PCCM, physicians must
- participate in CII/CPAR
- have a minimum panel size of 500
- work a minimum of 400 hours over 40 weeks per year
PCCM panel calculations can be viewed on the AMA Member Dashboard (member login required). In some cases, a physician's PCCM panel size will differ from their CPAR panel size, or the panel size calculation associated with the Panel Management Support Program (PMSP).
Before moving to the next step in the application process, learn how your PCCM panel size is calculated.
Alberta Health provides data updates to the AMA on a monthly basis. If your panel size has recently increased to exceed 500 and your data has not yet been updated, we encourage you to check back later.
For more details on prerequisites for the PCCM, explore the Eligibility & Enrolment section of our FAQ.
-
Step 3 | Express Interest
Express your interest through your AMA Member Dashboard (member login required) – only qualifying physicians will have access to the Expression of Interest.
The PCPCM Expression of Interest is open to physicians who meet the eligibility criteria, as determined by Alberta Health data. If the EOI checkbox is not visible on your dashboard, it indicates that you do not currently meet the prerequisites.
Note: Submitting an EOI does not commit you to transitioning to the new model. We encourage interested physicians to participate in the EOI process and continue assessing the model’s suitability as additional resources and information become available. Once the PCPCM Application Form is obtained (see Step 4), physicians can conduct final evaluations before making a formal decision to enter the model.
-
Step 4 | Obtain PCPCM Application Form
In March 2025, eligible physicians who have expressed their interest will receive a Primary Care Physician Compensation Model Clinical Alternative Relationship Plan (ARP) Application Form via email from the AMA. This form is unique to you and cannot be shared with other physicians.
-
Step 5 | Obtain PCPCM Business Arrangement Number
To obtain a PCPCM Business Arrangement Number (PCPCM BA), physicians will need to fill out the Primary Care Physician Compensation Model Clinical Alternative Relationship Plan (ARP) Application Form and return it to [email protected].
-
Step 6 | Billing with your PCPCM BA
Physicians who receive a PCPCM BA can begin to bill services starting April 1, 2025.
After April 1, 2025, physician applications will be processed on an ongoing basis with implementation for approved providers beginning the first business day of the following month.
In addition to holding a PCPCM BA, physicians must maintain their FFS BA to claim services outside of the PCPCM (e.g., for care provided to patients without an Alberta Healthcare card, specialty focus areas and services provided in hospital).
For more information, see the PCPCM Operations Manual.
